Please make requests for Disclosure, etc., by following these procedures:
- Please submit a public identification document (driver's license, passport, health insurance card, copy of resident record (certificate of resident record entry), copy of family register (certified copy), alien registration certificate, basic resident register card, or individual number card) to verify your identity, along with the information necessary to identify the personal information subject to the request. For copies of resident records (certificates of resident record entry) and copies of family registers (certified copies), please submit the original issued within 6 months.
- A fee of 1,000 yen (excluding tax) will be charged for each request for Disclosure, etc. For requests from overseas, actual costs will be charged separately.
